Blood Sugar and Insulin Logs:

If you have diabetes, we are best able to assist you when you bring in detailed records of your blood sugars.

While our office has the ability to download your glucose meter via our computers, your written blood sugars and insulin doses (if applicable), allow us to help make the best decisions with you regarding your overall diabetes care.  

Your blood sugar log is the most important thing you can bring to your visit...

We understand and appreciate how much work it takes to keep these records for lengthy time periods.  

Thus, if otherwise not possible, please try to keep detailed records with frequent blood sugar testing in the 1 - 2 weeks prior to your upcoming appointment.
